While I can not be certain, from the video it looked to me like Carly either was pushed or was tripped. It is impossible to tell who did it. There were several people behind her.

Fortunately, Michelle Fields was taking a nap against the stage right underneath Carly - but now she's filing another assault charge against Corey.

My best guess is a broken heel, my second best guess is that the Cruz girls bumped into her. After she is down you can see Ted little girls occupying the same spot Carly fell from.
