Where does a person get all these things for free? If the policy has a deductible you haven’t reached does the chosen doctor have to accept you for these procedures or do you go somewhere else? How can this work?

Where does a person get all these things for free?

Insurance companies have to provide as part of ACA requirements. You go to your primary physician who is one of your insurance company's providers for an annual physical and you pay $0 and your insurance company pays whatever they have negotiated with your doc. The other items are either performed by your doc or referred out by him.

Those were the sweeteners. The bad news comes from higher premiums to pay for all of the "free" stuff.
