I have them both now. I stand corrected. They are both Certificates of birth. The Hospital one is signed by the administrator, nurse, and board of directors president. It has my footprints on it. This one was denied.
The other certificate was issued by the health dept. and is certified by the health district. It shows Mothers maiden name and place of birth along with Fathers name and place of birth. Both ages are also given. This one was accepted by the State Department
