Lots of choices but I think some of your criteria are a little to stringent.
You will not find accuracy in a compact or midsize ... you will be accurate enough in personal protection, ccw, or at home. (I still like the shotgun for home .. we have to many manneuverability problems. Para has a great mid size 45, based on 1911 .. ultra reliable, but too many parts for me and it gets lost in my meaty hands. (My wife carries it ..loves it)
Don’t laugh .. I CCW a full sized Colt 1911, and to heck with condition 2, or 3 for that matter. I stay cocked and locked all the time. Never a worry. The sear is blocked from releaseing the hanner, and unless the finger is on the trigger, unit in the hand .. It will not go off. I’ve seen numerous people AD by gently lowering the hammer (Cond 2) I do not have time to chamber a round when I need to draw.
Large? Yes. But I am a large man .. 6’6” and 317 stones. And large hands .. Only the large grips on a Sig elite fit .. And the sig makes a fine weapon, but old firendships die hard. I wear an IWB that carries in front ...in dockers and a golfshirt (tucked in) nobody knows.
317 stones? Do you realize that 317 stones is 4438 lbs? You ARE a big guy.
Wow! And I thought I was big! 317 Stone! That works out to 4438 pounds! What are you doing carrying a wimpy full sized 1911 concealed? I'll bet you can carry a Remington 870 concealed, along with several boxes of shells!
