There's been some exploration of that theme in recent years, but most superheroes are seen as adopting a "secret identity" in order to protect themselves and their loved ones. Some, such as The Hulk and Rose and Thorn, are acknowledged to be true MPD cases, though.

I think one needs a really tight plot not to have MPD just seem utterly ridiculous... which it shouldn't, since it's a real disorder but it always seems so cliche in stories.

And if they really wanted to protect themselves and their loved ones, they'd move to Montana and take up fishing. No evil supervillian would invade a city if there's no superhero waiting! It's against the rules
