To do the 25th, his cabinet needs to report to Congress that he is disabled.
If he notifies congress he is no longer disabled he is immediately back in office. If the cabinet then insists, Congress can remove him with a 2/3 vote in both houses.

Contrast that with impeachment, simple majority in the house, 2/3 of Senate.
