I couldn’t find an explanation of how underlying condition was defined or determined.
But I did look at the plot they show of the daily deaths in NYC. I checked my Wikipedia source and it appears to be accurate. At least they agree.
For me, that is very good news. I have considered NYC as far and away our best test bed for predicting the course of this virus and, in particular, for estimating the real mortality rate. They just have the most people per square inch and the most cases.

Also, the number of people who are over age 60 or have underlying conditions is not trivial. 16% of people are over age 65 (I couldn’t find the percent over 60). Obesity is a comorbidity; something like 75% of people over age 25 are obese. Another comorbidity that increases the severity of Covid-19 is diabetes, which affects about 8.3% of the population. The thing is, there is a significant portion of the population who are in a high risk category.

Cancer.
70 million Americans are obese
35 million Americans have diabetes
68 million have high blood pressure
Assuming overlap, that means one third of American adults have underlying conditions.
Something like 80% of American’s have some underlying condition to start with, and then NYC is not like most of the country in that people there live cheek by jowl in crowded apartment buildings, public transportation, restaurants, stores, streets, and parks, etc.
