I just did a quick search of T.H.

I wonder if the death certificate it is an old form that has both Territory of Hawaii and State of Hawaii. More weirdness.

Only trouble is that the form itself has “STATE OF HAWAII” in the upper left-hand corner, so when the form was printed it was recognized as being a state document.

Hard to say exactly what was going on, but there are several anomalies that make the document suspect to me.
