Only trouble is that the form itself has “STATE OF HAWAII” in the upper left-hand corner, so when the form was printed it was recognized as being a state document.
Hard to say exactly what was going on, but there are several anomalies that make the document suspect to me.

Just a guess, a forger probably wouldn’t know what T.H. meant either. Maybe copied from an older death certificate not knowing it negated the STATE OF HAWAII.
Just my two cents. I don’t think that Death Cert is any more legitimate than Obama’s birth certificate.
