When I received mine it was the orginal form filled in by my mother (I could recognize her hand writing)laid on the copier and copied onto the green basket weave paper. It also had some fields she didn't fill in. My name for one (as she must have not decided yet what to name me) This was TYPED in. Also her occupation was filled in by a different hand writing. My certificate was registered 14 days after I was born. It does have the raised seal and signature on it notarizing it is the original certificate on file.

The interesting thing about this one of McCain is that most of the typing is evident as an impression made through multiple copies. Some of it looks like it was done directly on the copy, not on the top layer.
