Are there any published standards for a religious exemption, in any context? I haven’t seen any.

Being a catholic, I’ve never heard of a priest being the one to issue a religious exemption affidavit.

In Texas here, every two-years I had to request the form from the state, fill it out, notarize, then turn it into the school. This was the only standard for religious exemption that I know of.
