Well, we just have to understand that these shackups are perfectly willing to marry but it just costs so darn much and, for reasons entirely unknown to progressivism, Obozo has not seen fit to include the cost of weddings, receptions, honeymoons, home purchases, car purchases, automobile repairs, food, heat, electric, gasoline, landscaping, home maintenance, household servants, and hot water heating and similar items in his noble Obozocare legislation although each of those items are vitally necessary to health and really ought (pursuant to progressive "ethics") to be paid for through employer provided health care insurance via HHS mandates.
And, as to your other point, isn't rump-ranging the essential "marital act?" How else would Adam and Steve "consummate" their marriage?

What will Obamacare do when Adam and Steve contract AIDS, MERSA, and Hepatitis B? How will Obamacare handle the costs of Adam and Steve when they can no longer work and are in a Hospice dying of these? How will the government pay for their funeral when they are dead at age 40 of these terminal diseases?
