The problem was that the operators made a big mistake instead of following proper procedures, calling in power from elsewhere in the system, which caused a problematic surge/imbalance. It all happened in 9 seconds.
Operators made big mistakes? BULL! No human operaator could react in 9 seconds. It was all automatic and it worked like it was supposed to.
