And no, there's not always a cause. Stuff happens.

What is the reason the shuttle did not do what it was expected to? I am sure that is what TLBSHOW is asking for.

It finally wore out, as all things do? Tiles fell off just like they always do on the shuttle?

I'm not ready to say it was anybody's fault. It was an accident.
