The vaccine is generally safe for healthy people, and those who are healthy and had the vaccine before without complications are at the lowest risk of all.

That's no comfort to those that WERE healthy until given this "cowpox/horsepox grease/?" disease...

FWIW, complications could have been prevented in at least two and probably three of the individuals pictured above Proper modern dressings are very effective at preventing both transmission of live virus via scratching and/or viral shedding.
Proper screening of vaccine recipients is also necessary.

With the exception for epidemic, babies shouldn't get the vaccine. Immunocompropmised people shouldn't get it, and those with pre-existing skin disorders such as excema shouldn't get it.

You don't want the vaccine? don't get it.
