It's black, paperback, it says "Manuscript# 192111" in a box on the front cover. On the copyright page, it has this (centered of course, I'm too lazy to center it):

Here's some more info I found on the "Manuscript Copy" page: Manuscript Copy Copyright c 1980, 1981, 1982, 1983, 1985, 1986, 1988, 1989, 1990 by Neo-Tech Worldwide. First Printing was January 1980. My thirty sixth printing was December 1992. That's what I originally said "about 10 or 12 years ago". What was "BS" about it?
