You know, if custody is joint, it is only major bills which are to be split anyway. Parents would pay for all of the ordinary items that you mention, out of their own pocket with no need to account for such to the other parent, whenever the kids were with them. Major items, such as medical/school bills, would be what would come out of a joint CS account; those things are large, and easily trackable.

What about cases where one of the parents stipulates full placement to the other parent?
