My initial thought when I see claims that a handgun fired "without pressure on the trigger" is that someone had their finger on the trigger and simply pulled it without realizing it.

But some of these claims are that it fired while holstered, which is a very different thing. Not fired while holstering, or fired while unholstering, but fired while in the holster.
