During the filming of movies like "Ride with the Devil" from 1999, dozens and dozens of single-action revolvers were used in action scenes. Nobody was "accidentally" shot and killed. The link is to a scene in the movie where a revolver is drawn and cocked directly at another actor, pretty similar to what Alec Baldwin did when he killed the asst. director.
