It's highly unlikely that many people will say the greatest threat comes from the drug cartels that destroy the lives of tens of thousands of Americans each year. Deaths from illicit drug use can be differentiated in several ways, including an indirect risk factor for premature death from disease or injury, deaths directly attributed to drug overdoses, and violent deaths caused by clashes among gang members involved in the distribution of the lethal substances.

Drug use / abuse is a CHOICE. That problem will exist as long as morons CHOOSE to use drugs for other than medical conditions. No sympathy from these quarters.

But on the supply side, simply executing dealers (from kingpin down to street hustler) would put a dent into the ability to make that CHOICE.

China is targeting what it wants to eliminate—and making money doing it. At a lethal dose of just 2mg of very cheap powder of which it has exported tons to the US through Mexico, it could easily indiscriminately kill people through inhalation, consumption via water supply or skin absorption. But that hasn’t happened. Carfentanyl is even more potent.
