The commanding officer of the Navy warship that spilled thousands of gallons of fuel into a Virginia river earlier this month has been removed from her job.
Capt. Erica Hoffmann, commanding officer of the guided-missile cruiser Philippine Sea was relieved of her duties on Monday, Navy officials announced. Rear Adm. Richard Cheeseman, commander of Carrier Strike Group Two, lost confidence in Hoffmann's ability to command, Lt. Cmdr. Richlyn Ivey with Naval Surface Force Atlantic said.
"Many factors weigh into a decision to relieve a commanding officer due to loss of confidence," Ivey added.

Something is missing. COs don’t wake up, eat chow, and pull the lever, releasing fuel. There is a division officer, a deck officer and a chief that failed their CO.

One fuel spill does not torpedo the Captain's career. That's a screw up by someone actually handling the valves etc. You read them and their section leader the riot act, given them s**t duty for a month, pull their quals on a few things and have them (re)take training.
There must be more to the story - a history of significant mistakes (ie. the kind that get noticed by command) that indicate leadership failure from the top down.

The actual spill happened under the supervision of an enlisted sailor; officers (and especially captains) only give the orders rather than handling the hookups. So maybe you’ve got an E-2 or E-3 Seaman or Fireman doing the actual hands-on work, supervised by a third or second class petty officer, who is accountable to his division first class or Chief petty officer, who is accountable to the ensign, Lt. JG or lieutenant division officer, who is accountable to the XO...
A lot of people have to **** up for a captain to be relieved!

A Military Commander is 100% responsible for the actions of everyone under their command.

What about Engineering, Logistics/Supply, Maintenance and Deck officer?
She was on the fast track and in way over her head.
CO of a CG? That’s a big gig.
It often supports a Flag and leads an entire Surface Action Group.

I served 20 years and I said nothing dealing with responsibility.
