Do you know an American who is willing to live in and work irregular hours (like starting at 3AM)? I have a wonderful nanny but she would not live in and help with meals/housekeeping etc

yes I do know a few, and my wife did the same so there is no need for immigrants coming in for fancy, show off families to say I have an Au Pair.
