Are you talking about the US or a city in China where there are guards blocking the roads like something out of a TV show.
You need to stop thinking like a Westerner.
No one can get in or out of those cities, the Chinese usually cremate their dead so a body being taken to a crematorium would not seem out of place. But I, like you, have no idea what is happening in those cities in China.

I’m pretty sure China has death certificates, records files, halfway secure ID’s not too dissimilar to ours until recently, etc., at least in the more modern cities (and Wuhan definitely qualifies.) Heck, with socialized health care, they’d almost have to.
In a Tiananmen type situation where the PLA comes in and shoves out large scale protests, the local PLA forces, etc., killing many, who knows. Records might go bye-bye. I don’t think they’re to that level in this case tho’.
Likely there is CYA from officials, but that’s likely more on a Baghdad Bob level.
Ok, but, my points are:
1) I think that in ANY of the more modern Chinese cities (or significant size modern sections of most any of their large cities with such) this would have been picked up similarly. Maybe not QUITE as fast — Wuhan seems to be a sort of center of advanced medicine / research entirely aside from any military applications. (Granted that any center of advanced medicine would be a good place to recruit for and or place a level 3 or level 4 lab, with or not with any military apps. The reverse applies too.)
We have such places also: In my neck of the woods, if I fell ill with some new and / or killer disease, I’d prefer to be in the vicinity of the Vanderbilt University Medical Center.
I’m willing to bet that the MOST focus is in areas such as the one where they did that published (and fascinating) paper on local residents eating bats from a particular cave, and the researchers found human antibodies to the virus in the bats, in many of the residents. That said, a large outbreak is more likely in a city: The exposure is likely more diffuse, but applies to 3(?) orders of magnitude more people, in an urban lifestyle. Watch ‘em all as best you can...
Why Wuhan for this outbreak? Many possibilities. It is a transportation hub. The city is VERY spread out - lots of places for backyard animals, markets with live animals, and such. Caves: The province has lots of caves, and, therefor I’d expect: bats. Trip Advisor has a list of “The 10 Best Hubei Caverns and Caves” - Holy Crap! Some are spectacular! Just random chance. Etc.
2) It was our fellow FReeper Mariner who pointed out that the environment (human + Nature) in Chinese cities is better for cranking out outbreaks of new viruses than anything the best human researchers could create. We KNOW in a general way how this works, it is a natural occurrence, has happened repeatedly, and, I’ll say it again, humans have in Wuhan alone have provided Mo’ Nature with hundreds of thousands of nice incubators to work with. Then add in all the other cities. You are free to pursue the less likely cause, but I think it is counterproductive in terms of dealing with the present situation (basically, people freak out more if they hear “bioweapons”), and it almost guarantees the more fundamental changes needed in China to reduce the odds of a “next time” do not happen.
Frankly, I think it is a GOOD thing that the Chinese are studying these viruses, and more. They have some good researchers, and there is ongoing international collaboration. Sure, the labs are a risk. But it is a pipsqueak compared to what is brewing in their marketplaces, backyards, and at their dinner tables. THAT is what I can’t seem to get through to you.
With luck, the Chinese will not be devastated, and we will not be dented too badly, by this one. Maybe the next epidemic can be avoided. Everyone should pray for that.
