Either very small or badly made. Properly made, I’d guess one should have nearly cut him in half. Thank God he failed!
Looks like a very partial explosion with most of the material being spread around as dust.
Possibly a detonation of a primary charge (small pipebomb) with the secondary vest failing to react.
That spot in the center of a crowded concrete tunnel would have been optimal for damage had it worked. I think detonation was intentional.
Did that guy lose an arm? The pic of him lying on the sidewalk looks like it except there is no pool of blood.
It didn’t work at the right time. It was also a lot weaker than these things should be to kill a whole passel of people. He didn’t even successfully blow himself to Paradise.

So if it were to explode on the count of two, I think you would agree that it had malfunctioned.
