Its hard to figure how this could happen with radar on both ships screaming and alarming about a collision. Not if you knew the circumstances concerning an eerily almost (apparently) identical collision involving the USS PORTER in the Straits of Hormuz a few years ago. A solid projectile vomit head-shaker of disbelief. With that known history, this event falls in line with the others. Heads will roll.

It doesn't work that way near-shore. One could theoretically set alarms offshore but that's more of a yacht thing. Both ships should have had men on watch - rules of the road apply.
