I think it was either Quantim who is still around or Aphid who is not that did the photoshop. I think Aphid.
I think it is one of the most used photos that was photoshopped on the net that people think is actually real.
Found it. It was Freeper Aphid.

In case people don’t know, that is a composite done of 3 people on the steps of the Spanish Royal’s palace in Majorca.
From left to right: Carla Brunei, Princess Lucrezia of Spain (the Crown Prince’s wife) and Michelle Obama.
Note differences in posture, attire, grooming, hairstyle, shoes, etc.
As quantim says in his post: “One of these things, is not like the other...”
