Ive done long waits in the ER long before Obama came around. It all depends on the ailments youre competing with. Anything that looks like it might kill you in the next hour will always go through first. Visibly spewing contagions (coughing vomiting) are next. Then comes everybody else. It can be very frustrating. Its led to the rise of the quickmed/ not quite an ER places, very handy for folks that are sick but not ER sick, especially handy if you know what it is because its something you get a lot and just need somebody to write you a scrip. There used to be a lot of those places, then they went away, theyve come back a lot in the last few years.
Bleeding on the paperwork helps to get you seen quicker...:^)

Gasping out “I can’t breath” is a good one. Of course eventually they figure out you’re not on death’s door and you go to the bottom of all the stacks. But you get back there PDQ.
