Just FYI if you check the numbers less than 10% of all deaths are autopsied in the US

I'd say this would be a circumstance where an ME would call for an autopsy. It's not just "up to the family."

When my dad died (also overweight, 79, but hadn’t smoked in 20 years), the cops came and questioned my mom extensively.
She requested the autopsy to clear herself.
