As a few freepers have pointed to on the Canadian US consulates website it states the following: The child's parents may choose to apply for a U.S. passport for the child at the same time that they apply for a CRBA. Parents may also choose to apply only for a U.S. passport for the child. Like a CRBA, a full validity, unexpired U.S. passport is proof of U.S. citizenship. . On that same page you are instructed to click a link to find the instructions on how to apply for a first-time passport, where you find the following: What You Need to Apply A completed DS-11 form One 2âx2â passport photo Evidence of childâs U.S. citizenship Evidence of relationship between child and parent(s)/guardian(s) Parent(s) or guardian(s) identification and photocopy of identification Provide parental consent Pay applicable fee Then, there is this, scroll to page 2: http://www.state.gov/documents/organization/212239.pdf
