Indonesia was also the departure point for the A330 ( I think) bound for HKG. On the way it kept losing power and they finally had to declare into HKG. When they took the engines apart they found damage caused by little glass beads in the fuel. A search of the fuel tanks where they uplifted (Indonesia) found the contamination was rampant.
I agree with you; lack of oversight is dangerous and can be blamed on the Government..
