They probably put him in the cell with the tunnel already in place. When they got wind of the extradition request in Mex City, someone there probably got a nice 7 figure bonus for telling them it was time for him to leave, if he had not already been using the tunnel to come and go at will anyway.

Yep. This sort of thing isn't at all uncommon for Latin America. Pablo Escobar built his own prison and hired his guards. He had a helicopter for his own use right there in the courtyard.
