Where is his lawyer, and why isn’t he/she fighting this?

Great question! Hopefully, the lawyer is hard at work on an appeal.

“I am requiring Mr. D’Souza to see a new psychological counselor...and to continue...weekly consultation not as part of his punishment or to be retributive."

By what reasonable interpretation of his authority? What restraints or limits are there to this seemingly extra-judicial authority? What does that perceived authority portend for others who appear in his court?

The first thing the lawyer should do is petition for the judge to remove himself from the case. His statements have already impeached him.
