On second thought, perhaps we could provide a preprinted form to just sign instead. We could call it the Darwin Candidate Application Report Form. It would include a box for name, date, location, some check boxes for the reason for the for the shooting, and a line for the officer performing the service to sign off on the completion of the service.
Make it an NCR form with duplicate pages to submit and it could double as a voucher for reimbursement for the ammunition expended. Minimize the filing of Workers' Compensation for carpal tunnel syndrome from the writers' cramp. The officer can keep a pad of them with him like a ticket book and just submit them at the end of shift. Just leave the yellow copy tucked on the body and go on his way. No down time; no lost productivity.
