Its gone beyond that. Went to a new doc on referral. Got this questionaire: One section dealt with alcohol. Question, do you consume alcohol and if so how many drinks a week.
Next question....if yes, have you considered quitting.
I stopped at that point and asked the receptionist, what whether I have a drink has to do with my foot problem? Did not get a satisfactory answer, returned the forms and left.
Interesting fact: My internist recommended a drink a glass of red wine a day to promote blood flow.
I’m going to a Women’s Clinic today for first time....assuming I’ll have the questionnaire to fill out....I’m going to play games with them....hahahaha
Just don’t answer the questions. They USUALLY do not insist.
I always leave them blank and if questioned tell them it is not their business.

If you can, stay away form the corporate medical system. If you’re concerned with privacy, or concerned about pharmaceutical bias, see an independent physician that does not use networked electronic medical records and pay cash.
Many of the most experienced physicians are getting out of corporate medicine and going to all cash practices to avoid the bureaucratic and regulatory hassles. Unfortunately, over time many of these physicians will be retiring and the new ones coming up are being brainwashed into accepting fascist health care.
