If she died from the very strain in the vaccine, no one dropped the ball. The shot just didn’t work.

She could have had it already.

If she died from the very strain in the vaccine, no one dropped the ball. The shot just didnt work. Or more accurate, perhaps her immune system didn't work. It may also be possible (I didn't read the full story) that she was exposed to the real flu too soon after receiving the vaccine. If that happened, her body may not have had time to fully develop antibodies that could fight off the flu.

I was speaking in general terms, not specifically to this case. I didn't see when she received the vaccine and how much time had elapsed between receiving it and falling ill. It's possible she already had the virus in her when she received the vaccine. It's also entirely possible that the specific vaccine lot was compromised or the single multi-use vial being used to administer vaccines was compromised. It's also possible that there was a medication error of some kind. We don't know.
What we do know is that this is one single incident. Many millions of other people have received the vaccine and have not died from influenza.
I have always had mixed feelings about mandatory vaccinations for a number of reasons. I went ahead and got the shot though because my employer would have forced me to wear a mask around during my 12 hour shifts if I hadn't and I really didn't want to do that. I have had three viral infections since getting the shot but no influenza. I'm currently fighting a viral upper respiratory infection in fact. That's how nature intended it.
