I would give the easiest answer since it wouldn’t be under oath.
But shocking that they’d expect to use doctors as spying devices.
We are but units of measure to them - taxpaying devices, voting devices, who, when we are no longer of use to the state will face expulsion from the state - via our “health” care system. Of course....this will all just be for the benefit of the whole. /s

I had to go to the ER few months ago and the clerk asked me that question and I said NO. I could tell the clerk was embarass. He told me “I am sorry to ask you this but I have to”.

I was asked this last year by my daughters Pediatrician during a survey. I said Yes but what I should have said was “it's none of your business” but I was caught off guard. Yes even I can be caught off guard.
