Isn’t the Captain automatically relieved of command over something like this?

Yes...the Captain has the ultimate responsibility...but usually it's someone under him that neglected to do or see or read or calculate something.

My bro was on watch and using binoculars, he spotted a submarine periscope waaaaay off in the horizon on their course. Said it scared the sh** out of him.

Just about. I’m sure it ranks up there in the top 3 of ship Captains worst nightmare I bet. He’ll be Captaining a Tug or Garbage Scowl.
