Places like the Mayo Clinic may likely still be able to provide top level care for foreigners with cash and I would see other large clinics and even solo practice physicians moving to wholly private pay services. To do this they would have to opt out of all government and private insurance, but given the levels of reimbursement and control over care that would be imposed by Obamacare that might be an advantage. One result of Obamacare will be the creation of a two tier health care system... one for those that can afford it which provides current state of the art medical care and a government run system that provides second rate care for everybody else. If Obamacare outlaws wholly private pay services in the US look for these private pay clinics to spring up in Mexico, the Caribbean and even India.

Thailand. India. And probably after awhile, maybe a very short while, to Central America. England’s best phhysicians moved their practices to Asia and some Central American countries have changed their laws to accommodate free market medicine for foreigners. Cruise ship companies will start advertising for medical vacations. Costs will be low enough that middle class and even lower middle class folks will be able to contemplate the tours. I also expect that with the US formal entry into socialized medicine will come border and travel controls at least partly to inhibit people from trying to extend their lifespans beyond what is decreed by the bureaucracy. The ruling class will, of course, be excepted.

My impression is that all cash only services will be prohibited, that treatment will be only what the bureaucrats decree and only by approved assigned physicians and paid for only by approved "insurance plans."
