Intriguing, TigerClaws.

“Good comment though on Smoking Gun: No one has noticed the “M” in P.M. is a small font of a capital M. That would have been impossible on a manual typewriter. The keys could have created only a capital ‘M’ or small ‘m’, but not a smaller version of a capital M. Just sayin’.”

No. For time the form has a build in small “M.” The clerk just typed in “A.” or “P.” Makes sense if they use it on every form.

That small cap M probably was probably pre-printed on the form so that the nurse, or clerk, would only have to add A or P for AM or PM. I still think the form supplied is a fake.

.
Not true!

I used to have a typewriter whose lowercase keys were just small uppercase versions. (FWIW)
