No. For time the form has a build in small “M.” The clerk just typed in “A.” or “P.” Makes sense if they use it on every form.
look at post 17 it looks like they all had the small M

That small cap M probably was probably pre-printed on the form so that the nurse, or clerk, would only have to add A or P for AM or PM. I still think the form supplied is a fake.

Not true!
I used to have a typewriter whose lowercase keys were just small uppercase versions. (FWIW)
