Supplemental information is needed for LATE and/or AMENDED birth certificates, and that last line on this form has to be filled out if it is late or amended.

Every birth certificate is an abstract - there is a lot of information that is collected for health statistic reasons. No one’s birth certificate has ever shown all the information on the Certificate of Live Birth that the hospital filled out.
