HA! Ok I have the same lady on the phone I spoke to last week, she is checking the Rev. Number and what it is and means. I have sent her a scanned front and back of my COLB and she is going to be investigating this in the department for me.

ALSO, she is sending me the RECEIPT that has my moms name on it AND the DATE it was ordered. Take that!

You have no IDEA how much I will enjoy posting THAT document!

So at this point, the State of Hawaii is now involved in explaining my COLB.
