I just spoke with a lady in Vital Records (at 808 586 4539) who told me that she believes the designation in the lower left corner --which I read to her-- is signifying that is the date November 2001 when the form was revised which is used to print the requested data of the individual. The 11/01 signifies the month and year that the revised form begins being used for official documentation. I asked if there could be a document sent out in 2000 which would would carry the 11/01 designation and she said that to best of her knowledge the blank forms are printed prior to the date (11/01) but are put into service on the designation date and all previous versions of the form cease to be used and are destroyed.

This is not the same individual to whom I spoke yesterday afternoon when I called (808 586 4542) about the supposed destruction of paper documents that have been digitized for speedy accessing. Yesterday's call had an incredulous clerk questioning where I would hear such a notion since Hawaiian law requires that 'paper documents of all sorts --she named several types including birth and death records, etc-- which are digitized must be retained in archives and maintained into perpetuity'

Thanks for the info. I do not believe that the clerk is correct on the revision number. Mostly because I got my document in 2000.

I am NOT going to back down from the truth.

I have been trying all day to get through to the department. I either get a busy signal, or get lost in the automatic system, which has happened twice.

I repeat, I am not goin gto back down from the truth. My COLB is a document that was ordered in Feb of 2000 by my mother for me. I received it in Mar of 2000. The COLB has a date on the back of it that reads Mar6. 2007 The clerk, and I have her name, could not tell me how this could be, and was willing to send me a new COLB upon sending in my current document.

I can NOT explain the discrepancies. ALL I can do is to continue to state the truth, and REFUSE to back down from the truth.

Sohhh, you are saying, “11/01” is the same as “November 2001” and all other forms used before “November 2001” would not have this “rev” number?

Did she indicate the practice of rolling the date forward, on the stamp machine, by say, 6 years was common?
