Obviously, someone is grossly misinformed (or lying). Wonder who, which and why?

I can come up with two explanations:

1) someone received a document later than they remember receiving it
2) ... well, I've already stuck my foot out on this one so I don't feel like getting both feet off the ground, because there might be a third alternative which I have yet to fathomed.

I am convinced from what the Vital Records lady said (and I gave the phone number so anyone can call and ask the same questions) that the 11/01 designates the date on which the revised form went into use for printing CoLBs. It is not likey that it would have been used a year earlier than it went into use.
