The y chromosome. I don’t feel any need to encourage this poor person’s delusion. She will always be a “she.”
There may be more to it genetically than just the y chromosome, but I will grant you it is an imperfect transition. Still, I think she will be happier, although I doubt her level of "happiness" is one I would be satisfied with.
