1) Yes, his stepfather filled it out.
2) Legal name change was already done in Hawaii when his birth certificate was re-issued and the old one sealed.
3) He was registered as a muslim IN A CATHOLIC SCHOOL, I doubt if his father was grooming him to be a little muslim in a Catholic school.
4) The significance of this along with the other missing documentation is huge. Dual citizenship makes him inelligible to be Pres.

No it doesn't. Not being a native-born US citizen would make him ineligible, but having US and some other citizenship would not.
