It is called accountability. It is something that is a foreign concept to our political bureaucracy.
It could have been a mistake by a QM3 (E-4), but the Captain is still responsible.

Not just the Captain. The OOD and the conning officer might as well start putting together their civilian resumes.

They “might” get promoted sometime in the future, but this will be in their service jacket, on the OERs and will be red flagged on any selection boards they are ever in front of.

Short version? They’re history.

The word "accountability" doesn't exist in the Democrat Party.

Yes, and it provides some extra assurance that if we ever have a ship going aground in combat situations, we can be sure that the captain will be inexperienced in it.


Hmmm...I wonder...why does responsibility stop with the captain? Why not right up the chain?
