What changes in the conditions did he fail to recognize and get beyond? I think that it's pretty obvious that the "Mission Accomplished" phase ended and we moved into an occupation/nation-building phase, where our primary combat element was counterinsurgency.

Well, yeah! And both Rumsfeld and the Army have met that counterinsurgency. So the changes did change, he recognized it, and shift tactics accordingly.
