missing person was found dead in the rubble of the refinery explosion. Godzilla found the info.

Looks like an accident. We'd have fewer of these 'accidents' if we'd build more refineries so the ones we have wouldn't be pushed so hard.

The one loose end of the incident seems, to me, to be the fact that the first reports affirmatively stated that a person, who had checked out, was missing. This person was then said to have returned to the scene and perished in the fire/explosion.
Why would a worker check out and then return to the work area on his own time?
All in all, I tend to agree with you, and strongly agree with your approach to mitigating a continuing problem.
