I think he means that Islamic terrorists' only critical city is Mecca, Medina, and the end of Islam. They would still be terrorists but they would lose their support base as Islam was discredited in the eyes of all future generations. It is a nuclear option should the unthinkable occur in an American (or Allied) city.
