Remember that the smallpox vaccine is made using a live attennuated form of the vaccinia virus, NOT the smallpox virus. Vaccinia causes a very mild pox illness related to cowpox. It isn't possible to get smallpox from the vaccine because there isn't any smallpox virus present, although you can get vaccinia and it is somewhat contagious (and is very very rarely serious). Any live virus vaccine poses some risk of infection in an immunocompromised host - that's why there's no way they can use live smallpox virus in the vaccine.
